(6). Insurance binder

(Full coverage) Max $ 500.00 Deductible. We can

provide on the spot insurance.

(7). Copy of phone bill

- Landline can be in any name at same address.

- Cell or prepaid must be in customers name.

(8). Seven references ( Form will be provided).

(9). Check payable to commonwealth of Pennsylvania.

(10). SSI income and pension income need a copy of

letter.
